Care Coordination (CCP) and Cluster partners work with our families. The CCP consists of agencies that provide mental health services using a wraparound philosophy to children and families enrolled in Cuyahoga Tapestry System of Care. Cluster partners are community-based settlement houses agencies. These agencies provide family advocacy as well as support activities.
