Services & Support

Tapestry serves children and youth who are involved in either the child welfare or juvenile court systems.  A social worker or probation officer makes the referral to Tapestry which them starts the enrollment process.  An initial assessment is completed with the family to ensure that Tapestry is a right fit for the child and their family.  That initial assessment is followed by the development of a crisis plan (if needed), and then the family is put in touch with professional helpers who can support them in convening a team and helping the family develop their plan of care.   

The services that Tapestry provides are organized by “Clusters”, which are service areas that link to geographic districts established by the county’s child welfare agency.  Each Cluster has a designated provider for care coordination services, as well as a collaborative group of community agencies that provide family advocacy.  The family is connected to professional helpers in the Cluster closest to their home community.  The map below gives you a picture of where the different Clusters are located and which agencies work in the different areas:

In addition to the care coordination and parent advocacy services, families enrolled in Tapestry can also access something called wrap supports.  These wrap supports are available to help the family in accomplishing their plan or care and can include things like music lessons, respite, and camp.  A family’s Care Coordinator can help them identify and access these wrap supports.
